I think it's only the first time, after then you get a supply of some sort. Think I got fiber from one.

Only melee right? I beat him by throwing the cards that give me additional cards, but spawn on his board. Then I throw the ice weather and fatique him in the first round (lose a lot of cards). Then I can win the 2 last rounds easy.

Not sure, what it was i had no plan for it
BTW it was the master armorer, which you don't get until you complete a level 24 mission so doubt you would have done that yet.

About difficulty i can see why the gamespot reviewer bumped his normal difficulty playthrough up to hard half way through.
Tbh once you reach the early to mid 20s on normal unless you fight something your not ready for i.e. don't have the stuff its weak against or its several levels above you it can be a bit easy.
I don't mind it so much atm as i'm really just trying to enjoy the world, story and everything else in between. But it does render the idea of being a Witcher a bit of a joke as preparing for monsters isn't as important as it would be on hard.
I can definitely see Blood and Broken Bones, being a more pure experience and probably the " right way" to play the game and as such i do play to replay on that difficulty eventually.

Story missions? *bleep* loads, then there's all the side missions which link together with the story which without you are not getting anywhere near the same experience.
