Pretty much all the goals were caused by poor defending.

Players are to blame but not only them. Using a double pivot of Ramires-Meireles - having both Essien and Mikel fit and available - was a stupid idea from the start.

None of them are good enough defensively to perform the role and it was felt as they couldn't offer the back four enough protection or cover for the fullbacks.
