About to lose to Newcastle at home, they will trail Newcastle and Tottenham by four points and won't make it into top-4.

Real Madrid finished 5th in the year they won the UCL in 2000. Liverpool was like 30+ points behind the leaders when they won theirs. Yes, it is surprising Chelsea are in the final but it isn't something out of the ordinary.
Also, they can win the UCL although what happen with Liverpool and Everton wouldn't happen again...

They finished 5th in the league but UEFA granted them a spot in the CL.
It happened because UEFA did not want a situation where the Champions could not defend their title for the first time ever. They changed the rule that season and made it part of the rules from then on.
The CL spots for a country = Champions + remaining spots(if one of the clubs emerge champions)

The FA did not want to include Liverpool in the CL spots despite being champions, that's why UEFA stepped in and changed the rule.

Um, it actually happened before Liverpool. Real Madrid didn't qualify via league and won it in 2000. The issue was left for the LFP to solve and they said Real Madrid would go to the UCL while Zaragoza (who finished 4th that year) would go to the UEFA Cup.
The Liverpool one was different as the FA wanted both teams to go through and typical FA left it for UEFA to decide. Not to seem harsh on either, they gave the green light for both teams to go through. Needless to say, if Everton had beaten Villareal, it would have been an outrage...
