Anyways: Cannibal Holocaust and Salo: 120 days of Sodom own this thread. I think it's along the lines of the Serbian film, but I haven't watched it.

Cannibal Holocaust is so bad, it's banned in many countries because it's mistaken for a snuff film.

The scene where a man is killed using the blunt end of beer bottle to smash his face in during Pan's Labyrinth is disturbing, however I generally don't watch these sorts of films, so I don't have much to offer.

Scalping scene in Inglourious Basterds caught me off guard as well. Absolutely no warning that it was about to happen.
